In order
to expedite
your registration at the tournament headquarters you need to
make certain that you have the following information:

Copies of team’s State Roster (You
can email us *)
·
Guest player forms approved by your State Association
·
2 copies of Tournament Roster
·
Travel permit (for out of state teams only)
·
2 Copies of player’s
Illinois Medical waivers (No originals)
·
Player’s passes
Each team must submit all of the
above documentation. -
Please, bring with you to the registration on Friday night,
4 thru 10pm:
